Office Policy

Appointments that are not confirmed and or canceled 24 hour prior to appointment dates will be charged a $20 fee. Patients will be responsible for $20 fee at the following appointment.

Lab Policy: Routine labs must be done a week prior to the appointment. Please note that it is the patient responsibility to confirm which lab takes your insurance.

Prescription and Refill Request: If you request a prescription refill or to change your current medication please allow 24 hours for you request to be processed.
